Bereavement support
Example 1: Mortuary viewing rooms - Some excellent objective images from the work place included in this evidence piece. The candidate has explained how the process of viewings are managed within the department (e.g. notice board) and recorded (e.g. spread sheet). They have then supplemented the evidence work with some photos of the viewing room and how practice varies for young adults, children and paediatric viewings. There was an additional photo (not included) on this original evidence piece of viewings in practice during the Covid pandemic (i.e. glass screens in place). Finally, the candidate has broken up the text a regular points with images to make reading easier.

Example 2: Local SOP relating to viewings - the candidate has taken key pages from the SOP to copy and included in their evidence piece (rather than the whole SOP). They have then highlighted key areas (in bold text) to explain further the format of the SOP and then collated key information about the process. This not only shows that the candidate is aware and has read the SOP but also demonstrates understanding of the viewing process.

Example 3: Bereavement care support leaflets - Candidate has taken shown the front page of local leaflets available to the bereaved and then briefly highlighted and explained key areas contained within each - they have also annotated some of their work that again highlights key information.

Example 4: Viewing disclaimer form - this documents demonstrates some of the local mortuary governance relating to viewing of the deceased [patients] in their care - in particularly if the deceased is deemed unsuitable for viewing. As well as the local disclaimer form included, the candidate has also discussed strategies for effective communication that they would use in these situations, resulting in a piece that covers a number of learning outcomes. Candidate has also annotated the local form to explain key areas to the assessor (this also further personalises the evidence piece).

Example 5: Religion and culture arrangements - The candidate has included objective evidence (local leaflet) of information available to support staff to help raise awareness in supporting the bereaved with different faiths and beliefs. The candidate has also discussed why it is important to treat everyone in their care with respect and dignity.

Example 6: Viewing rooms floor plan - the candidate has drawn a floor of their local viewing area to identify key areas and explain how each functions in practice. The original evidence piece has been countersigned by the candidates work based mentor - this confirms the piece is authentic. This particular evidence piece will cover a number of learning objectives for this unit.
