RSPH Level 3 and 4 Diploma APT
All candidates: Please complete the form below for your preferred exam venue location.
The candidates chosen location will remain in place for the entire academic year – there is no need to resubmit this form once completed unless you wish to change venue.
If you require an exam resit then you must confirm your attendance with the North Tees Centre. Failure to notify us of your attendance in advance will result in in a non-attendance note for your result.

If no preferred venue is received by a candidate, then it will be assumed the candidate will be sitting their exam at the North Tees Centre based in Stocking-on-Tees.
